Information Collection Notice: CSNSW Sports – student participants

  1. We collect personal information, including sensitive information, of primary and secondary school students enrolled in Catholic schools in NSW (student/s) involved in CSNSW activities, as well as the personal information of their parents, carers or guardians before and while the student is involved in our activities.
  2. The primary purpose for collecting personal information is to provide a sports service to students, including staging sporting events and selection trials, and administering sporting pathways for students to progress to and earn selection in representative sports teams. We also collect personal information to perform necessary administrative tasks associated with our primary purpose.
  3. Personal information collected by us includes: names; contact details; date of birth; gender; and details about disabilities, illnesses and injuries
  4. Some of the personal information we collect is to satisfy our legal obligations and to enable us to discharge our duty of care.
  5. We may ask students and parents, carers or guardians to provide health information (which is sensitive information under the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs)) such as medical reports, and information about illnesses, injuries and disabilities.
  6. We collect personal information directly from students and parents, carers or guardians (for example, by way of online and hard copy forms, correspondence and in person), and we may also collect personal information from other schools, diocesan Catholic Education Offices (CEO) or Catholic Schools Offices (CSO), government departments, other organisations providing services related to sport, and medical and health practitioners.
  7. We will only use or disclose personal information where necessary to carry out our functions and activities, or to fulfil a legal obligation. We may disclose personal information and sensitive information to:
    a.Schools and staff at those schools (independent, Catholic systemic and public),
    b.Diocesan CEO or CSO
    c.Government departments including the NSW Department of Education
    d.Medical practitioners
    e.People and organisations providing support and health services to us
    f.People and organisations providing administrative and financial services to us
    g.Other organisations providing school sport services
    h.Organisations providing facilities, such as sportsgrounds and venues, for use by us
    i.Anyone authorised by you
    j.Anyone to whom we are required by law to disclose personal information to, including under child protection laws.

  8. Personal information collected from students is regularly disclosed to their parents, carers or guardians.
  9. On occasions information such as sporting achievements and results may be published on our website and in other publications we provide. This information may include photographs and videos of students’ activities. We will obtain written consent before publishing that information from students’ parents, carers and guardians and/or from the student/s.
  10. We may use an online or ‘cloud’ service provider to store personal information and provide services to us that involve the use of personal information, such as relating to email and messaging. Such personal information may reside on a cloud service provider’s server located outside Australia.
  11. Students, parents, carers or guardians are not required to provide us with their or their child's personal information, but if they do not, they may not be permitted to participate in some of our activities.
  12. Our Privacy Policy can be accessed on the CSNSW website (https://www.csnsw.catholic.edu.au) for more information about how we handle personal information.
  13. Our Privacy Policy contains information about how an individual may access personal information held by us and seek the correction of such information as well as how they may make and how we deal with privacy complaints. Access may be refused in certain circumstances, with such refusal notified in writing and with reasons where appropriate.
  14. If you provide us with the personal information of others, such as other family members, doctors or emergency contacts, we encourage you to inform them you are disclosing that information to us and why, that they can request access to and correction of that information if they wish and to also refer them to our Privacy Policy for further details about such requests and how we otherwise handle personal information we collect and complaints we receive.
