COVID 19 Exemption Form

For athletes who have followed the representative pathway and are registered for a MacKillop, Polding or NSWCCC selection event but cannot attend due to self isolation in accordance with the NSW Health Order.

Forms must be submitted and an sms sent before 9am of the first day of the event.
